Crestline Paving & Excavating Co., Inc. worksite located on Coder Rd. (South of 20A), Maumee, Ohio: On or about April 12, 2012, the employer did not ensure employees were protected from cave-in/collapse hazards while installing precast storm sewer basin in a trench approximately 12.00 feet deep. The south end of the trench was near vertical exposing the employees. No end plate was being used. B. Crestline Paving & Excavating Co., Inc. worksite located on Coder Rd. (South of 20A), Maumee, Ohio: On or about April 11, 2012, the employer did not ensure employees were protected from cave-in/collapse hazards while installing precast storm sewer basin in a trench approximately 12.00 feet deep. The south end of the trench was near vertical exposing the employees. No end plate was being used.

A. Crestline Paving & Excavating Co., Inc. worksite located on Coder Rd. (South of 20A), Maumee, Ohio: On or about April 12, 2012, the employer did not ensure a ladder was located in the trench box while an employee was working in the trench box. The trench was approximately 12.00 feet deep with unsafe access/egress. Crestline Paving & Excavating Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651, which was contained in OSHA inspection number 311609119, citation number 1, item number 2 and was affirmed as a final order on April 22, 2010, with respect to a workplace located at Toledo, OH.

The trench box was approximately 4.00 feet above the base of the trench exposing the employee to crushing injuries resulting from the trench box shifting vertically. B. Crestline Paving & Excavating Co., Inc. worksite located on Coder Rd. (South of 20A), Maumee, Ohio: On or about April 11, 2012, the employer did not ensure the trench box was less than two feet from the bottom of the trench. The trench box was approximately 4.00 feet above the base of the trench exposing the employee to crushing injuries resulting from the trench box shifting vertically.
