29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about February 28, 2023, at jobsite 2042 Marengo Ave Toledo, Ohio 43614, the employer Crestline Paving and Excavating Co, Inc. failed to protect employees from cave-in hazards in that employees were working inside a 6 ft deep trench performing waterline replacement/installation without cave-in protection installed.
