29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: Location: Pad Room, 12 Park St, Webster MA On 4/10/12 and at times prior, the fenced gate to the Wrapping Machine was not fixed or secured exposing employees to the hazards of operating machinery. On 4/10/12, an employee was seriously injured after gaining access to the machine through that unsecured gate.
