29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails: On or about 04/11/12, and at times prior thereto, employees were exposed to a fall hazard while using the stairs to access the work platform for the spreader tables, the stair rail system did not have intermediate rail installed.

29 CFR 1910.305(a)(1)(i): Metal raceways, cable armor, and other metal enclosures for conductors were not connected to all boxes, fittings, and cabinets to provide effective electrical continuity: a) On or about 04/11/12, and at times prior thereto, employees were exposed to an electrical hazard while working near the Spreader table #2. The 1/2" EMT conduit that supplies electricity to the "blower starter" was disconnected at a coupling. b) On or about 04/11/12, and at times prior thereto, employees were exposed to an electrical hazard while working near the Gerber #2. The 1/2" EMT conduit that supplies electricity to the "blower starter" was disconnected at a connector/junction box.

This disconnect was missing a 1" knockout on the side of the box, exposing live parts.
