All guards and other safety devices, including starting and stopping devices, were not properly maintained: Production line, Southwest cutter #7, on or about July 17, 2014, and at times prior thereto, employees operating the fabric Gerber cutter #7 were exposed to a struck-by hazard from the automated bridge fabric cutter head that does not have a functioning emergency stop.

29 CFR 1910.305(g)(1)(iv)(A): A flexible cord was used as a substitute for the fixed wiring of a structure; Fabric lifting station, South side, on or about July 17, 2014, and at times prior thereto, employees were exposed to an electrical hazard from a flexible cord that was being used to power a Dayton Electric Hoist (800 lb. capacity) and was hung over a metal beam in such a method that it damage the outer jacket.
