29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns. On April 12, 2012 an employee was working below other workers without wearing a hard hat, exposing the employee to overhead struck-by hazards. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c)

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about April 12, 2012 employees were on a roof performing carpentry work without fall protection exposing the employees to a fall hazard of 20.2 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
