29 CFR 1926.404(b)(1)(iii): Where ground-fault circuit interrupters were not utilized, the employer did not establish and implement an assured equipment grounding conductor program on the construction site covering all cord sets, receptacles which are not part of the building or structure: South side of the well site: On or about September 18, 2013, employees were constructing the piping used to connect the well to a gathering system. An employee used electric hand tools supplied from a welding machine through 3 wire cord sets without any ground fault protection.
