29 CFR 1910.178(q)(7):     Industrial trucks were being placed in services after the examination showed condition(s) adversely affecting the safety of the vehicle:      a)  In the dry section of the plant: On April 12, 2012 and at times prior to; the employer failed to remove an industrial truck from service after the inspection revealed that the back-up horn was inoperative and the rear view mirrors were broken, exposing employees working in the dry section to struck-by and crushing hazards.

29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders was not located away from elevators, stairs, or gangways where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:      a)  Compressed gas storage area in the pipe shop area: On April 12, 2012 d at times prior to, the employer failed to store compressed gas cylinders in a manner that would prevent them from falling exposing employees to struck by hazards when accessing the pipe shop.      b)  In the power recovery area: On April 12, 2012 d at times prior to, the employer failed to store compressed gas cylinders in a manner that would prevent them from falling exposing employees to struck by hazards.
