29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:  a)  #2 Soap tank;  On or about June 28, 2015 and at times prior thereto, the employer exposed employees working on the platform or the #2 Soap tank to fall hazards of up to 61 inches (approximately 5 feet) in that the ladderway entrance was not equipped with a swing gate or was not offset in order to keep a person from walking directly into the opening.
