29 CFR 1910.23(c)(1):     Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    a)  Moulding Rack; On or about April 12, 2012 and at times prior thereto the employer exposed employees to fall hazards of 6 feet 11 1/4 inches in that employees were allowed to walk and work on a platform that was not guarded by a standard railing which included a mid rail as required by the standard.

29 CFR 1910.23(d)(1):     Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails:    a)  Loading Dock Exit Door; On or about April 12, 2012 and at times prior thereto the employer exposed employees to fall hazards in that employees were allowed to walk up and down a set of fixed stairs with four or more risers that did not have a standard railing on both open sides.

29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      a) Machinery throughout the facility; On or about April 12, 2012 and at times prior thereto the employer exposed employees to potential hazardous energies in that employee were allowed to perform maintenance and servicing on a variety of wood working equipment at the facility without having a lock out program, without being trained in lock out tag out, and without machine specific lock out tag out procedures.

29 CFR 1910.213(c)(1):     Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    a) Shop Area - Delta Unisaw 10" Tilting Arbor Saw; On or about April 12, 2012 and at times prior thereto the employer exposed employees to amputation hazards in that employees were allowed to operate a Delta table saw without a guard in place that had an automatically adjusting hood and completely enclosed the portion of the saw above the table and above the material being cut as required by the regulation.
