29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)  On the east end of the machine area:  On August 14, 2014 and at times prior to; the employer failed to develop specific lock-out, tag-out (LOTO) procedures for each machine such as, but not limited to the KVAL, frame assembler that operates on 230/480 VAC, electrical and 90 psi, pneumatic, exposing employees to electrical shock hazards during weekly maintenance activities.  (b)  On the east end of the machine area:  On August 14, 2014 and at times prior to; the employer failed to develop specific lock-out, tag-out (LOTO) procedures for each machine such as, but not limited to the KVAL, casing cutter that operates on 440 VAC, electrical and 95 psi, pneumatic, exposing employees to electrical shock hazards during weekly maintenance activities.  (c)  On the east end of the machine area:  On August 14, 2014 and at times prior to; the employer failed to develop specific lock-out, tag-out (LOTO) procedures for each machine such as, but not limited to the Triad Cutter that operates on 440 VAC, electrical and 100 psi, pneumatic, exposing employees to electrical shock hazards during weekly maintenance activities.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):   (a)  In the warehouse section: On August 19 and at times prior to; the employer failed to remove a forklift from service in that was unsafe in that the back-up alarm was not operating, exposing employees working around the forklift to struck-by and crushing hazards.
