29 CFR 1910.212(a)(5): Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch.      Fans throughout the facility used for cooling purposes were observed missing the guard or having openings larger than 1/2 inch:    a) Mechanic shop; Fan had a 5 x 6 inch opening in fan guard exposing a belt and pulley mechanism and the fan blade.    b) Machine shop:           1.) Fan in lathe area had 3 x 1 1/2 inch openings in the fan guard.        2.)  Fan in Michael Montero's work area was missing the guard that covers the fan blade.    These conditions exposed employees to amputation hazards.

This partition shall be without openings and shall be gastight.      Throughout the facility the employer had oxygen and acetylene stored together on carts. The cylinders were not seperated by a noncombustible 30 minute fire rated partition or by at least 20 feet. Storing the cylinders together exposed employees to fire/explosion hazards.
