This violation occurred on or about April 10, 2012, in the kitchen area where a duplex outlet was not grounded. b) The employer does not ensure that all portable electric fans are effectively grounded. This violation occurred on or about April 10, 2012, in the kitchen area where an electric fan was not effectively grounded.

29 CFR 1910.305(d)(2): Panelboard enclosures. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons. a) The employer does not ensure that electrical panelboards are properly dead fronted. This violation occurred on or about April 10, 2012, in the electrical room next to the kitchen area, where a breaker panel that is used daily to turn equipment on and off during wash down activities was missing three (3) breakers which exposed interior energized components.
