29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s).    On or about 11/19/2013, the employer did not ensure that a fork lift used inside of the plant was tagged indicating that it has been removed from service until its operational status was determined. By not tagging the fork lift out of service, any employee could potyentially use the defective equipment.

29 CFR 1910.305(b)(2)(i): Covers of outlet boxes having holes through which flexible cord pendants passed were not provided with bushings designed for the purpose nor did the holes have smooth, well-rounded surfaces on which the cords could bear.    On or about 11/19/2013,the employer did not ensure that flexible cables entering junction boxes through the covers were within bushings as the cable passed through the door. A cable was observed entering a junction box of a conveyor belt section without a bushing which allowed moisture/water to enter the box, potentially creating shorting, arcing and electric shock conditions.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed to prevent moisture or water from entering and accumulating within the enclosures:    On or about 11/29/2013, the employer did not ensure that electrical boxes located on equipment in the Single Serve processing area were able to prevent water and moisture from entering the various boxes. Covers on the electrical boxes listed below allowed water and moisture to enter the boxes due to the gaskets becoming hard and brittle. This condition allowed for the equipment to become energized due tothe water and moisture buildup within.    INSTANCES:    a)    Mobile conveyor belt section. Small junction box on the length side of the belt.  b)    Large junction box at the end of the Mobile conveyor belt section  c)    Junction box located on the stainless steel sealer.  d)    Junction box on the hopper.
