29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers identified for the purpose:    On or about April 13, 2012, the covers for the conductors for the MCC trailer were not installed.  They were located inside the trailer in a table drawer.  This condition exposed worker to 480 VAC main power to the rig and an hazard of electruction.  The opening was on the side of the MCC trailer exposed to the generator house.

Types S, SC, SCE, SCT, SE, SEO, SEOO, SJ, SJE, SJEO, SJEOO, SJO, SJT, SJTO, SJTOO, SO, SOO, ST, STO, and STOO flexible cords and Types G, G-GC, PPE, and W flexible cables shall be durably marked on the surface at intervals not exceeding 610 mm (24 in.) with the type designation, size, and number of conductors:    On or about April 13, 2012, a SPT type cord without a grounding conductor was used the energize a refrigator.  Food was being stored in the refrigator, located in the mud house.  This condition exposed workers to the hazard of electrocution.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    On or about April 13, 2012, the strain relief devices were not effective creating an electrical hazard to workers:         a.  Black cord on the top row of outlets third from the right, located on the rig floor.  110 VAC cord had the outer sheath pulled back to a point the clamp would not seat.         b.  Black cord to well logger's trailer, located on the transformer provided power for the trailers.  110 VAC cord was too small for the strain relief device.         c.  Entrance cable to panel mounted to company man's trailer did not have a strain relief device.    Employees were exposed to hazards from electrical shock.
