29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: At the Rig 11 drilling site at Hannah-Roy Trust 17-11-20-1H well in Beckham County, Oklahoma, on or about April 13, 2012 and at times prior thereto, the Haws portable eyewash was empty at the caustic and lime mixing hoppers on the mud pit. Nomac Drilling Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.151(c), which was contained in OSHA inspection number 313371692, citation number 01, item number 003, and was affirmed as a final order on 04/08/2012 with a final abatement date of: corrected during inspection with respect to a workplace located at Brushes Fork Road, Buckhannon, WV.

29 CFR 1910.1200(g)(1): The employer did not have a material safety data sheet for each hazardous chemical which they used: At the Rig 11 drilling site at Hannah-Roy Trust 17-11-20-1H well in Beckham County, Oklahoma, on or about April 13, 2012, and at times prior thereto, the employer did not have material safety data sheets for employees who were using hazardous chemicals including, but not limited to, flammable Sigma Coatings Thinner 91-88 (xylene, ethyl benzene) for cleaning brushes and thinning paint and two-component coating Sigmadur 500 Base (xylene, ethyl benzene) and Sigmadur 500 Hardener (hexamethylene diisocyanate) for coating operations on the rig. Nomac Drilling Corporation Rig #50 was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(g)(1) which was contained in OSHA inspection number 313028284, citation number 01, item number 001, and was affirmed as a final order on 07/21/2010 with a final abatement date of 07/05/2010 with respect to a workplace located 7.5 miles West of Hwy 175, Mansfield, LA.

29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter: At the Rig 11 drilling site at Hannah-Roy Trust 17-11-20-1H well in Beckham County, Oklahoma, on or about April 13, 2012, and at times prior thereto, respirator fit testing was not provided for employees who were required to wear 3M 8247 R95 filtering facepiece respirators while adding bags of dry chemicals to mixing hoppers, including, but not limited to, caustic soda, caustic lime powder, MILGEL and MIL-Bar (containing crystalline silica, quartz), and MIL-Cedar Fiber.
