29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear, with no obstruction across or in aisles that could create a hazard. At Rig #30, on or about April 10, 2012, at the top of the stairway leading up to the rig floor, an air hose laid across the entire width of stairway causing a trip/fall hazard. Exposed to trip/fall hazard of 29-feet to the ground were six employees working on and around the rig floor.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Rig #30: On or about April 10, 2012, a 400 watt Appleton Area Master Flood Light attached to light pole #20 at the Shaker Area showed signs of damage. Employee(s) were exposed to a potential electrical shock hazards due to the following instances: a. The strain relief/insulation that prevents conductors from directly transmitting to joints or terminal screws had pulled apart 4 to 6 inches from the device. b. The exposed white conductor had a cut in its insulation.
