OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. The employer did not ensure that the employees were protected from fall hazards. On or about May 18, 2012 on Rig #6 located at Hughes 49 #2 Howard County, Texas, employees were exposed to fall hazards while working from the derrick of a rig where the employer: a) had not rigged the self-retracting lifeline to prevent its contact with hazardous objects which could damage the equipment or prevent its proper functioning; b) did not have the self-retracting lifeline inspected by a competent person at least annually; c) did not provide employees training to inspect, use, store, and maintain the equipment prior to each use; d) did not assess the effectiveness of user training of the equipment at least annually.
