29 CFR 1910.217(d)(1)(i): The employer did not use dies and operating method(s) designed to control or eliminate hazards to operating personnel on mechanical power press(es): a) EBC Industries - 1325 Liberty Street, Erie, PA / On or about April 16, 2012 an employee was injured from broken tooling while operating a Bliss Mechanical Power Press. The press at the time of the incident did not utilize a complete die set of both upper and lower tooling.

29 CFR 1910.217(d)(6): All dies did not meet the tonnage, stroke, and weight designation as specified in the 29 CFR 1910.217(d)(6)(i) through 29 CFR 1910.217(d)(6)(ii): a) EBC Industries 1325 Liberty Street, Erie, PA / On or about May 15, 2012, the upper tooling used for the Lollipop part was not marked/stamped with identification and die use information. In addition, a system was not established to ensure the correct tooling was used in the press for the part.
