EBC Industries - On or about 5/14/14, personnel performed servicing and maintenance operations on machinery/equipment that included but was not limited to forges and mechanical power presses. Periodic inspections were not conducted with all authorized employees. Periodic inspections were not conducted with employees that included but were not limited to those personnel who worked on second shift.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: A.) EBC Industries - On or about, 5/14/14, employees worked with and around corrosive chemicals, including, but not limited to, acid. The emergency eyewash station in the lab was not inspected and activated on a weekly basis.
