OSH ACT of 1970 Section (5)(a)(1):    The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:      a.) Etzel Masonry, Inc., 100 Campus Drive, Grove City, PA  16127, Rathburn Hall, north side of the structure, on or about April 10, 2012 and times prior thereto: employees were not provided with a load chart, capacity information, and operation/maintenance information for rough terrain forklift.  Employees operating the SkyTrak 8042 rough terrain forklift were not provided with a load chart and a user's/operators manual.   Load chart was missing from the Rough Terrain Forklift.  The users/operators manual was not in the Rough Terrain Forklift and/or on the job site.  Condition potentially exposed personnel to struck-by hazards from falling rough terrain forklift loads.
