1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    The employer allowed an employee to work in an unprotected excavation that measured approximately 6 feet in depth, 10 feet in width and 21 feet in length.  The south excavation side wall and end walls were vertical and were composed of clay like soil of type B consistency but the soil had been predisturbed by existing utility installation degrading it to class C.  No benching, shoring or sloping of the excavation walls had been done along the south, east, and west sides.
