29 CFR 1910.151(c):     Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:      On or about 18 April 2012, at the CARRIAGE SQUARE facility located at 4009 Gene Field Road, Saint Joseph, Missouri, employees were exposed to injurious corrosive materials including Ecolab brand Neutral Cleaner and Disinfectant, Ecolab brand Laundri Destainer, bleaching agents, and other chemicals, but suitable eyewash stations were not provided within the work area.

29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    On or about 18 April 2012, at the CARRIAGE SQUARE facility located at 4009 Gene Field Road, Saint Joseph, Missouri, employees were exposed to chemical hazards due to the fact that the workplace written hazard communication program did not contain a list of the hazardous chemicals known to be present at the facility, the methods the employer will use to inform employees of the hazards of non-routine tasks, or how the criteria specified in 29CFR1910.1200(f)-(h) will be met.
