29 CFR 1910.147(c)(1):  The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about April 17, 2012, at 512 Fresno in Fort Smith, Arkansas, the employer had not developed an energy isolation program for employees required to service or perform maintenance on equipment such as but not limited to the Kval   Door Mortise Machine and Interior & Exterior Door machines.  This condition exposed employees to struck by and caught   in hazards if the machines and/or equipment were unexpectedly energized.

29 CFR 1910.1200(e)(1):  Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:    On or about April 17, 2012, at 512 Fresno in Fort Smith, Arkansas, a Hazard Communication Program had not been developed for the benefit of employees using and/or exposed to hazardous chemicals/ingredients such as but not   limited to wood dust and fiberglass; toluene, methanol, acetone, MEK, and 2-Butoxyethanol contained in the Crown   Lacquer Thinner; toluene, acetone, hexane isomers, and N-hexane contained in the the Wilsonart 110 Solvent; and, acetone, n-Pentane, toluene, cyclohexane, and N-hexane contained in the Wilsonart 500 Contact Adhesive.

29 CFR 1910.1200(h)(1):  Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area.  Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals.
