29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) Jobsite: On or about April 4, 2012, two employees were not protected from falling while they were working from a roof. The employees were bringing in rolls of carpeting that were being lifted to the roof level.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l): Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178. (a) Jobsite: On or about April 4, 2012, an employee was operating a JLG lull and he was not trained to safely operate it. The employee was using the JLG lull to lifit rolls of carpeting up to a roof.
