29 CFR 1910.147(c)(4)(i):   Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)   In the test cell area, procedures were not developed when performing tasks such as modifying and adjusting of the engines during the testing stage of production.

29 CFR 1910.147(f)(3)(i):   A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group.  (a)  Testers and modification employees would perform servicing on engines when they did not pass the parameters during the test cycle.  Group lockout/tagout was not performed where each employee would apply a lock to ensure no accidental/inadvertent restart of the engine.

29 CFR 1910.147(f)(4):   During shift or personnel changes, specific procedures were not utilized to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout devices between off-going and oncoming employees, to minimize exposure to hazards from the unexpected energization or start-up of the machine or equipment, or the release of stored energy.  (a) During shift changes, employees would verbally inform or write on the whiteboard what energy sources were turned off during the servicing.

29 CFR 1910.147(c)(7)(i)(A):   Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  (a) Testers and modification employees would perform repairs on engines in the test cells to obtain the necessary specs in order to ship the product.  The employees were trained to the affected employee level even though work on the engines was being performed.  THE GE ENERGY - WAUKESHA GAS ENGINES WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.147(c)(7)(i)(C), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 122393069, CITATION NUMBER 01, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON AUGUST 24, 2012 WITH RESPECT TO A WORKPLACE LOCATED AT 101 W. ST. PAUL AVE., WAUKESHA, WI.
