29 CFR 1910.147(c)(1):     Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a. Entire facility - On or about March 14, 2012, the employer had not implemented an energy control program for employees who perform servicing and maintenance on such equipment as the shot peening machines.  Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.

29 CFR 1910.147(d)(4)(i):     Lock out or tagout devices were not affixed to each energy isolating device by authorized employees:  a. Job Shop - On or about March 14, 2012, employees who performed service/maintenance on the shot peening machines did not affix a lockout or tagout device on the 3 inch gate air valve.  Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.

Entire Facility - On or about March 14, 2012, the fabrication shop employees were required to operate powered industrial vehicles.  The employees did not receive training on the safe operation of powered industrial vehicles.

Fab Shop - On or about March 14, 2012, employees were required to operate the following bandsaws with the unused portions of the blades unguarded:  1. DoAll Vertical band saw  2. #1 and #2 Kalamazoo horizontal band saws  The employees were thereby exposed to the hazard associated with reciprocating saw blades.

29 CFR 1910.219(f)(3):     Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  a. Fab Shop - On or about March 14, 2012, the chain and sprocket drive on the Ursviken guillotine shear was not guarded, thereby exposing employees to in running nip point hazards.

29 CFR 1910.242(b):     Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  a. Fab Shop - On or about March 14, 2012, employees required to clean excess metal and plastic shavings from stock material used an unregulated compressed air line reaching 80 psi.  Employees were thereby exposed to the hazards associated with air pressure over 30 psi..
