29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Well site: On or about April 11, 2012, employees rigging down an oil/gas well drilling rig worked on elevated rig surfaces. In the following instance two employees did not use provided personal fall protection equipment and were exposed to the hazard of falling over four feet: 1) On top of the wire tray for the motor control center. 2) On top of the rig dog house.

29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: East side of the well site: On or about April 11, 2012, two employees rigging down an oil/gas well drilling rig worked on the shale shaker platform. The shale shaker platform had an unprotected opening in its guard rail system.
