This condition exposed the employees to caught-between and struck-by hazards.

The employees were exposed to electrical shock and/or burn hazards.

Drill Site: On or about November 14, 2017; both emergency lights in the VFD trailer had unprotected openings in the electrical boxes. This condition exposed the employees to an electrical shock hazards.

This condition exposed employees to electrical shock hazards.
