29 CFR 1926.451(c)(2):  Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.    JOBSITE:  The base plates of the fabricated frame scaffold from which an employee was performing stucco work, were not set on mud sills to prevent sinking and/or displacement.

29 CFR 1926.451(e)(1):  Scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. Crossbraces shall not be used as a means of access.    JOBSITE:  The fabricated frame scaffold platform, from which an employee was performing stucco work to the steeple of a church, was not provided with a safe means of access.

Such activities were not performed by experienced and trained employees selected for such work by the competent person.    JOBSITE:  The fabricated frame scaffold platform, from which an employee was performing stucco work to the steeple of a church, was not erected under the supervision and direction of a competent person.

29 CFR 1926.451(g)(1)(vii):  For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee wsa not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    JOBSITE:  The employee performing stucco work from the platform of the fabricated frame scaffold, was not protected from the hazard of falling 19 feet 6 inches to the ground by use of guardrails, personal fall arrest system or other equivalent devices.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    JOBSITE:  The employees, engaged in performing stucco work, were exposed to the hazard of falling between 10 feet to 15 feet to the ground while working on a steep (8 in 12 pitch) roof.

29 CFR 1926.1053(b)(6):  Ladders were not used on stable and level surfaces unless secured to prevent accidental displacement.    JOBSITE:  A Werner 8 foot step ladder, used by an employee to perform stucco work, was set on 2 x 10 planks that ran from a steep (8 in 12 pitch) roof to a fabricated frame scaffold, without being secured to prevent displacement.
