Worksite: The metal frame scaffold, from that employees were working and having the "ratio" of more than four to one (4:1) was not restrained to prevent from tipping by any means.

Worksite: The metal frame scaffold platform from that employees were working at a height of approximately 16 feet above the ground was not fully planked.  S & J Stucco, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.451(b)(1) which was contained in OSHA inspection number 395542, citation number 1, item number 1, issued on 9/11/2012 with respect to a workplace located at 283 Bridgeport Ave., Milford, CT and became a final order on 9/25/2012.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.   Worksite: Employees were "stepping up" more than three (3) feet from the ground to step on the bottom rung of the integarl laddrers of the metal frame scaffold to access upper levels.  S & J Stucco, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.451(e)(1) which was contained in OSHA inspection number 395542, citation number 1, item number 3a, issued on 9/11/2012 with respect to a workplace located at 283 Bridgeport Ave., Milford, CT and became a final order on 9/25/2012.
