29 CFR 1910.215(a)(4): Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage:   (a) In the woodworking shop on or about April 24, 2012, and at times prior thereto, the work rest was about one-quarter inch from the abrasive wheel on the right side of a 110 VAC Craftsman 8 inch bench grinder.

29 CFR 1910.215(b)(9): Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, were not constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel.  The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch:  (a) In the vehicle maintenance shop on or about April 24, 2012, and at times prior thereto, the tongue guard was about one inch from the abrasive wheel on the 6 inch Jet Equipment & Tools bench grinder, model number JB6-6A.  The grinder was used to sharpen shovels, screwdrivers and other metal parts.    (b) In the woodworking shop on or about April 24, 2012, and at times prior thereto, the tongue guard was about 7/8s inch from the left side abrasive wheel and about 3/4s inch from the right side abrasive wheel on a Craftsman 8 inch bench grinder.  The grinder was used to sharpen metal tools.

29 CFR 1926.253(b)(2)(ii): Assigned storage spaces were not located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:    (a) In the kitchen shop tool cage on or about April 24, 2012, and at times prior thereto, the argon and acetylene cylinders were stored on the floor next to a shop vacuum and were not secured.
