29 CFR  1910.36(g)(2): An exit access must be at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access.  Groundkeeper's Shop: Employees were expose to entrapment-related safety  hazards in case there is an emergency situation. The exit access was only 23 inches wide due to items parked in  the way including  a step ladder, chairs, a club car, and rock salt bags.

Carpenter's Shop: Employees were exposed to health hazards related to wood dust (Pine, Cedar, Plywood) due to the lack of a proper PPE hazard assessment.

29 CFR  1910.134(c)(2)(ii): The employer must establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily is medically able to use that respirator, and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user.   Carpenter's Shop: An employee was exposed to health hazards related to wood dust. The employer did not ensure that the employee is medically able to wear the respirator and proper respiratory protection training was not provided.

29 CFR  1910.213(a)(12): For all circular saws where conditions are such that there is a possibility of contact with the portion of the saw either beneath or behind the table, that portion of the saw shall be covered with an exhaust hood, or, if no exhaust system is required, with a guard that shall be so arranged as to prevent accidental contact with the saw.  Carpenter's Shop: Employees were exposed to safety hazards because the employer did not ensure that the moving parts of the Unisaw were guarded to prevent inadvertent contact while being operated. Employees were exposed to  amputation hazards from in-running nip points while engaged in wood working activities.

29 CFR  1910.1030(f)(2)(i):Hepatitis B vaccination was not  made available after the employee has received the training required in paragraph (g)(2)(vii)(I) and within 10 working days of initial assignment to all employees who have occupational exposure unless the employee has previously received the complete hepatitis B vaccination series, antibody testing has revealed that the employee is immune, or the vaccine is contraindicated for medical reasons.  Chukchansi Gold Resort and Casino: Employees were exposed to health hazards because  a Hepatitis B Vaccination was not made available to all employees who have occupational exposure to Blood Borne Pathogens.
