29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Well head: On or about April 17, 2012, employees rigging up a service rig in preparation of reworking an injection well were not using personal fall protection equiment. In the following instances this exposed them to the hazards of falls over 4 feet high: a) An employee standing on the stored work floor. b) Two employees working on the work floor. c) An employee standing on the power tong.

29 CFR 1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach: Rig working area: On or about April 17, 2012, employees rigging up a service rig in preparation for reworking an injection well used the rig winch line to lift rig equipment and structure. The chain sling attached to the winch line did not have an attached tag.
