Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm from fall from heights, fire and explosion hazards: a) On or about September 10, 2014, the employer allowed the use of an emergency escape system that was not installed per the manufacturers requirements. Employees were exposed to the hazard of fall from heights and inability to escape fire and explosions. b) On or about September 10, 2014, the employer allowed employees to conduct work activities from the derrick work platform without having access to the secondary exit route emergency escape device. Employees were exposed to fall from heights, fire and explosion hazards.
