OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: Well site: On or about April 18, 2012, an employee worked on service rig derrick mounted work positions during the repair of an oil/gas well. The emergency means of egress, the Geronimo system, provided for the employee's use was not readily accessable from one of the work positions, and its ground anchorage was insufficent. This exposed the employee to the hazards of fire and/or explosion, or falling from an elevation. The American Petroleum Institute Recommended Practices for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, RP 54, 6.10.1, requires that an auxiliary means of escape be installed on rig mast/derricks and that it provides a ready and convenient means of escape from the derrickman's working platform. Shebester Bechtel, Inc. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, SECTION 5(A)(1) OF THE OSHA ACT, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312381122, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 5, 2009, WITH A FINAL ABATEMENT DATE OF JUNE 9, 2009, WITH RESPECT TO A WORKPLACE HAZARD AT THE JOE 3-1 WELL SITE, SEC 3-6N-4W, MCCLAIN COUNTY, DIBBLE OKLAHOMA.
