29 CFR 1910.119(f)(1)(i)(B): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and addressing the steps for conducting normal operations: 1) On April 24, 2012 and at prior times, the employer did not develop and implement written procedures that provided clear instructions for clearing the condensate trap drain on the acetylene compressors. Employees were exposed to fire and explosion hazards due to the potential for acetylene releases.
