29 CFR 1910.119(d): The employer did not complete a compilation of written process safety information pertaining to the equipment in the process.                                                                    IN THE ALTERNATIVE        29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process.      On or about March 13, 2013 at the Western International Gas & Cylinders, Inc. facility in Bellville, Texas the employer did not complete a compilation of written process safety information that accurately represented the Acetylene Generation Process.    a) Process line continuations/transfers between drawings not present or not complete. For example the origination of 6"-CHWS-CS-NW700 on Drawing 19PIDAS009 cannot be located. as the transfer from drawing 16PIDCW010 cannot be found.    b) Set pressures for relief valves are not input on drawings. For example the set pressures for PSV 801 and PSV 802 on P&ID 12PIDDH027 shows a blank entries.    c) Detail  for the Scrubber S-711 and Heat exchanger HX-712 represented on P&ID 19PIDAS009 is missing and/or incomplete.    d) There is no relief pressure setting for the relief device RD-968 on scrubber S-711, shown on P&ID 19PIDAS009.     e) There is no relief pressure setting for the relief device RD-970 on scrubber S-811, shown on P&ID 19PIDAS010.    f) Detail  for the Acetylene Generator G-710 represented on P&ID 19PIDGN007 is missing and/or incomplete.    g) Detail  for the Acetylene Generator G-810 represented on P&ID 19PIDGN008 is missing and/or incomplete.

29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. The employer failed to document that process equipment in the Acetylene Generation Process complies with recognized and generally accepted good engineering practices. This condition was observed in the Acetylene Scrubber System in the Generation Process of the Western International Gas & Cylinders, Inc. production facility in Bellville, Texas, when the employer failed to document that process equipment complies with RAGAGEP when:    a)    The employer failed to ensure that HX-712 was manufactured in accordance with RAGAGEP such as ASME Boiler and Pressure Vessel Code Section VIII. Critical information, such as, but not limited to minimum wall thickness, maximum fluid temperatures and pressures, and code to which the vessel was built were not included in the process safety information for the vessel. The vessel does not have a nameplate attached.     b)    The employer failed to ensure that HX-812 was manufactured in accordance with RAGAGEP such as ASME Boiler and Pressure Vessel Code Section VIII. Critical information, such as, but not limited to minimum wall thickness, maximum fluid temperatures and pressures, and code to which the vessel was built were not included in the process safety information for the vessel. The vessel does not have a nameplate attached.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:    The employer did not completely enclose horizontal shafting to protect workers against exposure to rotating shafts on or about March 13, 2013 in the Acetylene Generation Area of the Western International Gas & Cylinder, Inc. facility in Bellville, Texas, when the shafts to the agitators in the G-710 and G-810 Acetylene Generators were observed to have sections exposed.
