29 CFR 1910.134(c)(2)(i): The employer determined that respirator use by employees was permissible and did not provide the employees with information contained in Appendix D of this section: (a) High Plains Construction Services, LLC @ 22777 WCR 31, La Salle, CO 80645: On and before 4/25/12, the employer did not provide an employee with the information that is contained in Appendix D. The employee, Mr. Steve Herrari, voluntarily wore an SAS Model 8611, N95 filtering face piece while operating the concrete crusher.
