29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    (a)  Near Aisle 10 on the North Dock, a cover was missing on the electrical panel exposing energized wires. Employees were exposed to electrical hazards.

29 CFR 1910.176(b):     Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:    (a) In the A-Aisle near door G, a rack beam was not attached to the column and hanging off the racking unit exposing employees to struck-by hazards.     (b)  In the A-Aisle, a rack column for AA.02.A through AA.02.F was damaged in that it was bent and twisted exposing employees to struck-by hazards.     (c)  In the A-Aisle, a yellow column beam(BB.03.A through BB.03.E) was not attached to the column in that it was suspended from one side, exposing employees to struck-by hazards.     (d)   In the A-Aisle, a rack beam supporting tractor brackets was deflected downward, exposing employees to struck-by hazards.     (e)  In the A-Aisle, an 8-foot pallet was broken and hanging downward over the front and back sides of the racking unit, exposing employees to struck-by hazards.         OAKLAND LOGISTICS, INC. DBA LINC LOGISTICS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD [29 CFR 1910.176(b)], WHICH WAS CONTAINED IN OSHA INSPECTION NUMBERS:  313525248, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON APRIL 15, 2010,   313528358, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 14, 2010,  AND 98569, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 21, 2011 WITH RESPECT TO A WORKPLACE LOCATED AT 7100 DURAND AVENUE, STURTEVANT, WISCONSIN 53177.
