29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: a) An employee who was not trained in the proper donning of a personal fall arrest system harness did not recognize the hazard associated with wearing the harness with the D-ring connection point below the shoulder blade area when working at a height greater than seventeen and one half (17.5) feet above compacted surface. b) An employee who was not trained in the proper use of a personal fall arrest system did not recognize the hazard associated with using a knot in a lifeline as a way to connect to an anchor point as part of a personal fall arrest system when working as a height greater than seventeen and one half (17.5) feet above compacted surface. c) An employee who was not trained in the proper use of a personal fall arrest system did not recognize the hazard associated with using a knot in the lifeline as a way to connect to the D-ring of his harness as part of a personal fall arrest system when working at a height greater than seventeen and one half (17.5) feet above compacted surface.
