29 CFR 1926.1053(b)(16):     Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately removed from service.    A) The aluminum extension ladder used to access the garage roof had the top right rail broken off.    B) The fiberglass extension ladder used to access the main house roof had splits in the lower parts of the top section rails.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)    A) Employees performing residential reroofing work were not protected from falling by complete personal fall arrest systems or other means.    B) One employee who was wearing a harness while working on the roof had a rope tied around his waist instead of connected to the dorsal Dee ring.    C) Three employees working on the roof were connected to one anchor point.    Town City Construction has been previously cited for a violation of this Occupational Safety and Health Standard, or an equivalent Standard, which was contained in OSHA Inspection #312557887, Citation #1, Item #1, which was confirmed as a Final Order on 11/15/10, with respect to a workplace located at W6653 Rickey Lane, Greenville, WI 54942.

Town City Construction was cited previously for a violation of this Occupational Safety and Health Standard, or an equivalent Standard, which was contained in OSHA Inspection #312557887, Citation #1, Item #2C, which was affirmed as a Final Order on 11/15/10, with respect to a workplace located at W6653 Rickey Ln, Greenville, WI 54942.
