29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: A. DM Masonry, 1205 Peach Street, Erie, PA 15601, east side of the structure, on or about April 26, 2012 and times prior thereto: Employees were performing masonry work on a Non-Stop Scaffold system 30 feet above ground level. An area between the building and the end rail on the working side of the platform was not protected with guardrails.

29 CFR 1926.451(h)(2): Employees working below a scaffold were not protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section:. A.) DM Masonry, 1205 Peach Street, Erie, PA 15601, south side of the structure, on or about April 26, 2012 and times prior thereto: Employees were performing masonry work at a height of 30 feet from a scaffold. An employee was working below and falling object protection was not in place.
