29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    On or about April 23, 2012, and at times prior thereto, employees operating a saw did not wear eye protection expsosing them  to the hazards of being struck by flying debris and broken saw blade fragments while operating a DeWalt table saw without a blade guard.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the employees wear eye protection while operating a table saw.

29 CFR 1926.304(i)(1):     Each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut:    On or about April 23, 2012, and at times prior thereto, an employee was exposed to the hazards of being struck by flying debris and broken saw blade fragments while operating a DeWalt table saw without a blade guard.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the guard is used on the table saw.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)    On or about April 23, 2012, and at times prior thereto, an employee was performing framing work on the joists of a residential structure at an approximate height of ten (10) feet five (5) inches without any means fall protection. The employer did not have a fall protection plan and did not provide fall protection training.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that fall protection is used for jobs conducted at hieghts of six (6) feet or greater.

29 CFR 1926.1053(b)(4):     Ladder(s) were used for purposes other than the purpose for which they were designed:    On or about April 23, 2012, and at times prior thereto, a six (6) foot folding ladder was propped up against the frame of the building exposing the user to a fall hazard.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure ladders are used properly.
