29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    At the job site, an employee was observed below workers actively installing shingles from the roof of a residential home without being protected by a protective helmet.  The eave to ground was approximately 23 feet, 5 inches where the worker was standing.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

The workers were exposed to a struck-by hazard.    Abatement certification and abatement documentation is required for this violation. Workers were exposed to a potential fall hazard.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. 29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    At this work site, employee's used an extension ladder to gain access to the roof of a single story residential home.  The ladder was raised approximately 1 foot above the roof's eave.  The ground to roof eave measured approximately 19 feet, 1 inch near where the ladder was stationed.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s)
