29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a)  1930 Devonwood Drive- Observed on or about 02/25/2021, employees in the process of roofing activities on a steep roof,  6 in 12 slope (vertical to horizontal), were exposed to a 10' 3" fall hazard, in that, fall protection equipment was not installed or utilized.  Lagunas Interstate Construction LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1326976, citation number 2, item number 1 and was affirmed as a final order on 08/03/2018, with respect to a workplace located at 420 Joseph Drive, Fairview Heights, IL 62208.  Omar Laguna was previously cited for a violation of an equivalent Occupational Safety and Health Standard, 29 CFR 1926.501(b)(11) which was contained in OSHA inspection number 1263760, citation number 1, item number 3b and was affirmed as a final order on 01/01/2018, with respect to a workplace located at 408 Johnson, Pocahontas, IL 62275.
