29 CFR 1926.350(a)(7): A suitable cylinder truck, chain, or other steadying device was not used to keep cylinders from being knocked over while in use. 29 CFR 1926.350(a)(12): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965. On or about 13 March at work site on 6204 Milwaukee Ave, compressed gas cylinder on north west side was not properly supported to prevent it from being knocked over, exposing employees to struck by hazard from high pressure projectile.

29 CFR 1926.351(b)(2): Only cable free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected shall be used, except that cables with standard insulated connectors or with splices whose insulating quality is equal to that of the cable are permitted. On or about 13 March 2012 at work site on 6204 Milwaukee Ave, employees had used a welding cable with several splices with in the 10ft (3m) of the holder exposing them to electrical shock or electrocution.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway was not at least 18 inches (46 cm) wide. On or about 13 March 2012 at work site on 6204 Milwaukee Ave, an employee was observed sitting on scissor lift guardrails making this a scaffold platform, in which a scaffold platform is a work surface, elevated above lower level. Guardrail would then have to meet the requirements of 1926.451(b) (scaffold platform construction). By bypassing the guardrail system the employee was exposed to a fall hazard of over 14 ft
