29 CFR 1926.1101(k)(8)(vii): After the building owner or employer identified previously installed presumed asbestos containing material (PACM) and/or asbestos containing material (ACM), labels or signs were not affixed or posted so that employees would be notified of what materials contain PACM and/or ACM, and/or the employer did not post signs in lieu of labels, and/or ensure, to the extent feasible, that employees who come in contact with these signs or labels could comprehend them:    a) On or about  3/10/12, the damaged Thermal System Insulation (TSI) on the hot water tank (Boiler room, Building 4) being repaired was identified as containing approximately approximately 20-25% amosite asbestos and 10-15% chrysotile asbestos, ABM Facility Services did not ensure that labels/signs identifying the asbestos in accordance with the OSHA requirements were posted so that the engineering crew would be notified of what materials in the room contained asbestos.  ABM employees were allowed to enter the affected area to perform work, on 3/18/12, 4/2/12 and 4/3/12, before remediation was completed on 4/6/12.

29 CFR 1926.1101(k)(9)(v): Training for Class III employees was not consistent with EPA requirements for training of local education agency maintenance and custodial staff as set forth in 40 CFR 763.92(a)(2).  Such a course did not include also "hands on" training and did not take at least 16 hours.    a) Where presumed asbestos containing material (PACM) and asbestos containing material (ACM) in Thermal System Insulation (TSI) and surfacing material was likely to be disturbed causing potential employee exposure during required maintenance and repair work on HVAC, steam, heating, mechanical and plumbing systems, and its components, in Boiler Room of Building #4, at the Kraft work site, 801 Waukegan Rd., Glenview, IL, ABM Facility Services did not provide the crew the required 16 hour "hands on" Class III training which included informing workers of the location of asbestos containing material, and training was not consistent with EPA requirements for training of local education agency maintenance and custodial staff as set forth in EPA 40 CFR 763.92(a)(2), and per the EPA 20T 2003 "Managing Asbestos in Place" July 1990 or its equivalent content.

29 CFR 1926.1101(k)(9)(vi): Training for employees performing Class IV operations was not consistent with the Environmental Protection Agency (EPA) requirements for training of local education agency maintenance and custodial staff as set forth at 40 CFR 763.92(a)(1):    a) Where ABM employees performed Class IV activities such as maintenance and custodial activities, in which employees may contact but did not disturb ACM (asbestos containing material) or PACM (presumed asbestos containing material), and activities to clean up dust, waste and debris resulting from Class III work done either in-house or by a contractor at the Kraft site, 801 Waukegan Rd., Glenview, IL,  ABM Facility Services did not provide training consistent with EPA requirements for training of local education agency maintenance and custodial staff as set forth in 40 CFR 763.92(a)(1), and per the EPA 20T 2003 "Managing Asbestos in Place" July 1990 or its equivalent content, including providing available information concerning the location in the work place of Thermal System Insulation (TSI) and ACM or PACM materials when performing maintenance and repair work, including work on HVAC, steam, heating, cooling and mechanical, and plumbing systems and components, on floor and ceiling tiles.
