29 CFR 1910.23(a)(4): Every skylight floor opening and hole was not guarded by a standard skylight screen or a fixed standard railing on all exposed sides:      a) ABM Facility Services, Vanguard Headquarters, Malvern - Employees, while performing emergency generator testing and HVAC maintenance activities on the building roofs, were exposed to fall hazards of approximately fifteen (15) feet.  The employer did not ensure that plexiglass roof hatches in Buildings 100, 200, 300, 400 and 500 were protected by skylight screens or standard railings on all exposed sides. Observed on or about March 19, 2014.      b) ABM Facility Services, Vanguard Headquarters, Malvern - Employees, while performing emergency generator testing and HVAC maintenance activities on the building roofs, were exposed to fall hazards up to approximately fifteen (15) feet.  The employer did not ensure that plexiglass skylights in Buildings 200 and 500 were protected by a skylight screen or standard railings on all exposed sides.  Observed on or about March 19, 2014.
