29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a)Glue Barrel: Employees were exopsed to a 6 inch long rotating shaft with four protruding 1/4 inch hexbolts. Machine guarding was not provided to protect employee from rotating shaft, as determined on April 20, 2012.

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to requirements specified in 29 CFR 1910.219(m) and (o): (a)Mezzanine Storage Area: A horizontal belt on a Dayton 24 inch diameter belt drive tubeaxial fan (Model #3C411A) was not fully gaurded. The belt was approximately 40 inches from the floor and the top of the guard was open leaving a 3 inch opening at the front and 1 1/2 inch opening on the sides, as observed on April 20, 2012.
